It's the MAIB, or the Marine Accident Investigation Branch of the British Ministry of Transport. They investigate all marine accidents, and publish annual reports for the learning value. The main site is at:

The incidents are classified as merchant, commercial fishing, and small craft. The small craft incidents can be pretty scary, like the sailboat skipper who went overboard with his boat aground with a line tied to his waist, intending to walk out a kedge, but got the line caught in the prop and was pulled underwater to be drowned. Lots of lighthearted stuff like that.
