Loss of situational awareness was the root cause. I wonder if those new female crewmembers are too distracting. Also, looking through the port to see what's out there seems to have gone out of style. Every navy ship going down the Straights has multiple radars going. Perhaps the Bridge staff forgot where the screens are located? Maybe they thought they were still tied to the dock. Kinda scary.
