How many ways can one simple pulley be installed wrong?

Came across this after the neighbor non-mechanical owner said he had trouble putting a flag up after changing the line. Thought it'd get a few chuckles here or maybe some eye-rolls.

1. It's obviously installed upside down.
2. The screw was probably used because the u-bracket is spread too wide for the clevis pin it came with. Once the top end was at the bottom and the spread out U-bracket squeezed back to shape with pliers, the clevis could be easily installed with the circular retaining ring. (You do carry spares, right?)
3. There's a plain nut on the screw which can back off.
4. The screw is long enough to catch clothes or skin.

All good points and a good chuckle. Worse yet, that vintage plastic sheave looks like UV has ruined it. I wonder why there was a desire for a block at the bottom when it belongs at the top? (He clearly does have a "shroud cleat" for the flag halyard to cleat to.)
